This fixes #12623 (bidirectionality breaks impredicativity).

Persistent arrays expose a functional interface but are implemented
using an imperative data structure. The OCaml implementation is based on
Jean-Christophe Filliâtre's.

Add headers to a few files which were missing them.

Kernel should be mostly correct, higher levels do random stuff at
times.

Note currently it's impossible to define inductives in SProp because
indtypes.ml and the pretyper aren't fully plugged.

We do up to `Term` which is the main bulk of the changes.

This completes the Loc.ghost removal, the idea is to gear the API
towards optional, but uniform, location handling.
We don't print <unknown> anymore in the case there is no location.
This is what the test suite expects.
The old printing logic for located items was a bit inconsistent as
it sometimes printed <unknown> and other times it printed nothing as
the caller checked for `is_ghost` upstream.

Some parts of Evarutils were related to the management of evars under constraints.
We put them in the Evardefine file.
