This lets us avoid passing ~ontop to do_definition and co, and after #10050
to even more functions.

We remove all calls to `Flags.is_program_mode` except one (to compute
the default value of the attribute). Everything else is passed
explicitely, and we remove the special logic in the interpretation loop
to set/unset the flag.
This is especially important since the value of the flag has an impact on
proof modes, so on the separation of parsing and execution phases.

This is a pre-requisite to use automated formatting tools such as
`ocamlformat`, also, there were quite a few places where the comments
had basically no effect, thus it was confusing for the developer.
p.s: Reading some comments was a lot of fun :)

We make `declaration_hook`s optional arguments everywhere, and thus we
avoid some "fake" functions having to be passed.
This identifies positively the code really using hooks [funind,
rewrite, coercions, program, and canonicals] and helps moving toward
some hope of reification.

In particular, we highlight the similarities and differences between
local and global definitions.

"Declaration" hooks can be polymorphic on their return type, however
this facility doesn't seem used in the codebase.
We thus remove the polymorphism with the hope to be able to reify the
control later on.

There can be a name clash if there's a module and a constant of the
same name. It is detected and is an error if the constant is first
but is not detected and the name for the constant not
registered (??) if the constant comes second.

to 8.7, it could also be triggered by the previous restrict_context I
think.

This reduces conversions between ContextSet/UContext and encodes
whether we are polymorphic by which constructor we use rather than
using some boolean.

This was (once again) a spurious inter-dependency, that we solve by
introducing a new module with the proper functionality. This helps in
cleaning up the code. Note that no code was changed, other than
removing the setting of the references.
